Lyra gives you and your dependents access to FREE mental health support when enrolled in Personify Health plans. Whether you need therapy, coaching, or tools to manage daily stress, Lyra makes care easy and personalized.

Hinge Health Virtual Physical Therapy
Hinge Health is a FREE guided physical therapy program designed to help reduce joint or muscle pain. This plan is available when enrolled in a Personify Health or Centivo plan.

Available July 1, 2026. For employees enrolled in the Personify Value Copay, Choice HSA, Premier EPO, or Centivo PCP Partnership medical plans.

Vida is a FREE virtual health program that helps manage health conditions like diabetes and co-occurring high blood pressure and high cholesterol.
If you’re managing type 2 diabetes and are enrolled in the Value Copay, Choice HSA, Premier EPO, or Centivo PCP Partnership plan, you can access the Vida Health Diabetes Management Program for FREE. There’s no cost to join—and you’ll get expert support right at your fingertips.
Considering a GLP-1 Medication?
Enrollment and ongoing participation in Vida are required for GLP-1 medications.
Your program includes:
1-on-1 support from Registered Dietitians and Certified Diabetes Care and Education Specialists to build sustainable diabetes management habits around nutrition, movement, sleep, and stress—plus goal setting, regular check-ins (video, phone, or chat), and ongoing accountability.
Vida offers a FREE Blood Glucose Meter, Blood Pressure Cuff, or Withings Scale based on clinical eligibility.

Midi Health Perimenopause and Menopause & Midlife Care
If you’re enrolled in a Personify Health plan, you and your eligible dependents have access to Midi Health, a virtual care program designed for natural perimenopause and menopause, as well as surgically induced or chemotherapy induced menopause.
Hormonal changes during perimenopause and menopause can impact sleep, mood, focus, weight, and overall quality of life. Midi Health connects you and your eligible dependents with clinicians who specialize in perimenopause and menopause and midlife care—so you can get real answers and a care plan built around you.

Care is provided through a specialized provider network and is available to employees and eligible dependents enrolled in a Personify Health plan; standard cost-sharing applies.

Cost to Join
You’ll pay an initial $28 enrollment fee plus your first two months’ membership dues ($28 + $28) at sign-up. After that, standard monthly dues apply and are billed directly through the program.
How to Access Active&Fit Direct
To enroll in Active&Fit Direct, log in to Workday and select the Benefits & Pay section. From there, navigate to your benefits and locate the Active&Fit Direct link to view program details and complete your enrollment.
